I think it is a fantasy for a lot of guys. When it really becomes interesting is when it is done in a way that is not part of the guy's fantasy. That is when it is fun to see (same with trampling). There are things that I enjoy, but the kicker in my mind is when it crosses the line of something that I do not enjoy. I like to see it go past that point and like to imagine it afterwards. So even though the fantasy is there, I desire it going past the fantasy point where the focus is totally on the sitter and the face underneath becomes nothing more than an object or a tool for the sitter. In that regards, it does not matter if it is done for reward or punishment, just as long as the sitter enjoys fully without regard to the person she is sitting on. Hard to explain :)
Maybe always a reward, but if the guy is denied then it's both?
